All new student registrations for the current or upcoming school year must be completed online.<\/p>\n

Required Documents Needed to Complete Registration<\/strong><\/p>\n

Current Mortgage\/Settlement Statement OR Lease Agreement<\/p>\n

Current Electric Bill<\/p>\n

Current Other Utility Bill (water, gas or cable)<\/p>\n

Driver\u2019s License (verification purposes)<\/p>\n

Student\u2019s Birth Certificate (original or certified copy)<\/p>\n

Student\u2019s Physical Health Form (dated within 12 months of enrollment)<\/p>\n

“Immunization Record” MUST BE<\/strong> on the Florida 680 Immunization Form<\/p>\n

Once you have completed the online registration process, you need to contact the school to complete the registration process. Please contact Carrie Ortbals, Guidance Secretary at carrie.ortbals@stjohns.k12.fl.us<\/a> or (904) 547-8410 to schedule an appointment to sit down with a guidance counselor and select classes.
