The school must have new Medical Management Plan forms on file EACH school year.

• Medication Authorization Form (all other medical conditions/needs)
• Medication Policy ** Please read prior to submitting medication and management plan**
• Over the Counter Medication (self-carry of non-prescription/OTC Medication)
